Transitional Care Administrative Assistant

Overview

Our client is looking for a detail-oriented Transitional Care Administrative Assistant to provide essential operational and administrative support for a Transitional Care Management (TCM) company. In this role, you will coordinate schedules, manage cross-provider communications, and maintain accurate patient records in compliance with healthcare regulations. You will serve as a key liaison between patients, providers, and healthcare facilities to ensure seamless care coordination and daily office functionality. Schedule:

  • Monday - Friday, 8:00 AM - 5:00 PM Central Time (40 work hours per week)

Responsibilities

  • Answer and direct incoming phone calls, emails, and other communications professionally.
  • Schedule patient appointments, follow-up calls, and staff meetings.
  • Maintain accurate electronic patient records and update documentation as needed.
  • Verify patient demographics, insurance information, and required documentation.
  • Assist with patient intake, referrals, and coordination of care activities.
  • Prepare, organize, and file medical and administrative documents.
  • Monitor incoming faxes, referrals, and physician orders, ensuring timely processing.
  • Coordinate communication between patients, providers, hospitals, skilled nursing facilities, home health agencies, and other healthcare partners.
  • Track Transitional Care Management (TCM) services, including post-discharge documentation and follow-up requirements.
  • Support billing and administrative staff by gathering required documentation for claims and reimbursement.
  • Order and maintain office supplies and equipment.
  • Prepare reports, spreadsheets, and presentations as requested by management.
  • Maintain confidentiality of patient information in accordance with HIPAA regulations.
  • Assist with audits, compliance activities, and quality improvement initiatives.
  • Perform general office duties including scanning, copying, mailing, and filing.
  • Complete additional administrative tasks as assigned.

Requirements

  • Minimum of 1–2 years of administrative or office experience, preferably in a healthcare setting.
  • Experience with Transitional Care Management, care coordination, medical office operations, or case management is preferred.
  • Familiarity with Electronic Health Record (EHR) systems is highly desirable.
  • Excellent verbal and written communication skills.
  • Strong organizational and time-management abilities.
  • Ability to prioritize multiple tasks and meet deadlines.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).
  • Experience using EHR/EMR systems and healthcare software.
  • Strong attention to detail and accuracy.
  • Professional telephone etiquette and customer service skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Knowledge of HIPAA privacy and confidentiality requirements.
  • Problem-solving skills and a proactive approach to administrative support.
