We are seeking a detail-oriented and highly organized Medical Documentation Assistant (Non-Voice) with a Registered Nurse (RN) background to support our Chronic Care Management (CCM) program. The ideal candidate will combine strong clinical knowledge with administrative expertise to accurately document patient interactions, maintain compliant CCM records, and support care coordination activities. This role is essential in ensuring timely, accurate, and complete documentation while enhancing continuity of care for patients with chronic conditions.
Schedule:
- Monday to Friday, 8:30 am to 6:00 pm (US Pacific Time) with a 30-minute unpaid break (40 hours per week)
Key Responsibilities:
- Prepare, review, and maintain Chronic Care Management (CCM) documentation and patient notes in accordance with organizational policies and CMS guidelines.
- Accurately document patient files, assessments, care plan updates, medication reviews, and care coordination activities.
- Update electronic health records (EHR) with complete, accurate, and timely documentation.
- Monitor monthly CCM documentation requirements to ensure compliance and billing readiness.
- Coordinate with physicians, nurse practitioners, and other healthcare providers regarding patient care plans and documentation.
- Identify documentation deficiencies and communicate with the clinical team for clarification when necessary.
- Maintain patient confidentiality and ensure compliance with HIPAA and other applicable healthcare regulations.
- Perform administrative tasks including scheduling, data entry, document management, and reporting.
- Support quality improvement initiatives by maintaining organized and audit-ready patient records.
Required Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 1–2 years of clinical nursing experience.
- Experience with Chronic Care Management (CCM), care coordination, or case management is preferred.
- Proficiency in Electronic Health Record (EHR) systems.
- Excellent written documentation and communication skills.
- Strong attention to detail and organizational abilities.
- Ability to work independently while managing multiple priorities.
- Proficiency in Microsoft Office Suite and general computer applications.
Preferred:
- Knowledge of CMS documentation and billing requirements for CCM services.
- Experience with remote patient care or telehealth documentation.
- Familiarity with medical terminology, ICD-10, and CPT coding related to CCM.
- Experience working with U.S. healthcare providers or medical practices.
