Job Overview
We are seeking a highly organized and detail-oriented Home Health Care Coordinator to support the daily operations of a privately owned home health care services agency. In this role, you will monitor multiple referral platforms, process new patient intakes, and maintain accurate records within the MyUnity EMR system. You will also be responsible for verifying insurance eligibility, reviewing clinical metrics, and generating daily operational reports. This full-time, remote position requires an active, proactive professional capable of managing fast-paced communication from Monday to Friday, 8 AM – 5 PM MST.
Schedule: Mon-Fri, 8 am - 5 pm MST with 1 hour unpaid break (40 hours per week)
Responsibilities
- Referral Management
- Continuously monitor multiple referral platforms during business hours (8:00 AM – 5:00 PM, Monday through Friday, excluding lunch), including Epic, Kaiser, CarePort, DINA/Tango, and other assigned systems.
- Check referral platforms at least every five minutes and respond to referrals within required service levels:
- Kaiser referrals immediately.
- Other referral sources within 5–15 minutes.
- Review incoming faxed referrals every 30 minutes.
- Accept or decline referrals according to company procedures.
- Archive faxed referrals following established processes.
- Email referrals and physician orders to the appropriate staff based on assigned locations.
- Medical Records & Data Management
- Accurately enter client information into the MyUnity electronic medical records (EMR) system.
- Maintain organized and up-to-date referral spreadsheets, tracking logs, and operational checklists.
- Audit medical records for assigned documentation requirements and report findings as needed.
- Access CORHIO when requested to retrieve client information and securely distribute records to the appropriate team members.
- Insurance & Authorization
- Verify insurance eligibility for new clients.
- Process authorizations accurately and efficiently to support timely patient admissions.
- Visit & Clinical Documentation Support
- Review, adjust, and process CellTrak visits according to company procedures.
- Monitor the SHP portal for:
- Start of Care (SOC)
- Resumption of Care (ROC)
- Patient discharges
- Review quality metrics related to the six improvement questions.
- Notify managers and clinicians when improvement measures are not met and follow up until corrections have been completed.
- Reporting
- Generate and distribute daily operational reports to assigned staff.
- Prepare daily and weekly reports on key performance metrics, including charting delays and other assigned operational indicators.
- Assist leadership by providing timely reports and tracking information as requested.
- Additional Responsibilities
- Perform additional administrative and operational duties as assigned after demonstrating proficiency in core responsibilities.
- Support process improvement initiatives and maintain compliance with company standards and healthcare documentation requirements.
Requirements
Required
- Previous experience in healthcare administration, medical records, referral coordination, patient intake, or a similar administrative role.
- Experience working with Electronic Medical Record (EMR) systems; MyUnity experience is preferred.
- Knowledge of insurance verification and authorization processes.
- Strong data entry skills with exceptional attention to detail.
- Excellent organizational and time management skills.
- Ability to monitor multiple systems simultaneously while maintaining accuracy.
- Strong written and verbal communication skills.
- Proficiency in Microsoft Excel, Outlook, and other Microsoft Office applications.
- Ability to handle confidential patient information in compliance with HIPAA and company policies.
Preferred
- Experience with Epic, Kaiser referral systems, CarePort, DINA/Tango, CellTrak, SHP, and CORHIO.
- Previous experience in home health or hospice operations.
- Experience generating operational or performance reports.
