Position Overview
We are seeking a compassionate and detail-oriented Remote Clinical Care Coordinator to join our growing healthcare team. This full-time remote position is ideal for a Medical Assistant, Licensed Practical Nurse (LPN), or Registered Nurse (RN) who is passionate about delivering exceptional patient care between provider visits.
The successful candidate will play a key role in monitoring patient health, assessing changes in condition, and communicating clinically significant findings to the supervising provider or clinical team to ensure continuity of care.

Key Responsibilities
- Monitor and maintain ongoing communication with patients between provider visits.
- Perform routine patient assessments and identify changes in health status.
- Escalate significant clinical findings or concerns to the supervising provider or clinical supervisor.
- Document patient interactions accurately and thoroughly in the electronic medical record (EMR).
- Support care coordination initiatives to improve patient outcomes.
- Provide patient education and reinforce care plans as directed by the clinical team.
- Collaborate with providers and other healthcare professionals to ensure timely, high-quality patient care.
Qualifications
Required
- Certification as a Medical Assistant, or an active LPN or RN license.
- Excellent verbal and written communication skills.
- Strong clinical assessment and critical thinking abilities.
- Comfortable working independently in a remote environment.
- Proficient with EMR systems and standard computer applications.
- Reliable high-speed internet connection and a dedicated remote workspace.
Preferred
- Experience with:
- Remote Therapeutic Monitoring (RTM)
- Chronic Care Management (CCM)
- Telephone Triage
- Bilingual in English and Spanish.
- Previous experience in outpatient, primary care, or care management settings.
- Previous pain management experience preferred
