Healthcare Referral and Intake Coordinator

Job Overview

We're is seeking a highly organized, compassionate, and results-oriented Healthcare Referral and Intake Coordinator to support a dementia care program focused on caregiver engagement, patient intake coordination, and appointment conversion.

In this role, you will own the full front-end referral journey—from receiving referrals and verifying eligibility to engaging caregivers, addressing concerns, scheduling appointments, and ensuring a seamless transition to the care navigation and provider teams. You will serve as the primary point of contact for caregivers and families, helping them understand the value of the GUIDE Model program while guiding them toward appropriate care services.

The ideal candidate combines empathy with strong communication and conversion skills, ensuring caregivers feel supported while consistently moving referrals toward successful enrollment and scheduled appointments.

Work Schedule: 40hrs per week, Monday to Friday 10 AM to 7 PM | Eastern Time

Key Responsibilities

Referral Intake & Coordination

  • Receive, review, and track incoming referrals from home care agencies, referral partners, caregivers, and other referral sources.
  • Ensure referral information is complete and follow up on missing documentation or details when necessary.
  • Conduct preliminary eligibility verification according to GUIDE Model program requirements.
  • Maintain accurate referral records and status updates within CRMs, EHRs, spreadsheets, or referral tracking systems.
  • Manage multiple referrals simultaneously while maintaining timely follow-up and documentation accuracy.

Caregiver Engagement & Program Education

  • Contact referred caregivers and families promptly in a compassionate and professional manner.
  • Explain how the referral was received through a home care agency or referral partner when applicable.
  • Introduce and educate caregivers about the GUIDE Model program and available support services.
  • Clearly communicate program benefits including:
    • Care navigation
    • Care planning
    • Caregiver education
    • Dementia-proficient provider support
  • Build trust and rapport with caregivers who may feel overwhelmed, hesitant, or unfamiliar with dementia care services.
  • Identify caregiver needs, concerns, barriers, and challenges through active listening and thoughtful questioning.

Appointment Conversion & Scheduling

  • Address caregiver questions, objections, and hesitations effectively and respectfully.
  • Convert qualified referrals into scheduled appointments with care navigators and dementia-proficient providers.
  • Coordinate virtual and in-person appointments as appropriate.
  • Confirm appointments and provide reminders to minimize no-show rates.
  • Follow up consistently until referrals are enrolled, scheduled, deferred, or closed.

Documentation & Team Coordination

  • Document all outreach attempts, scheduling updates, eligibility status, caregiver responses, and barriers to participation.
  • Provide warm handoffs to care navigators and clinical teams.
  • Escalate complex clinical, behavioral, or family-related concerns to the appropriate internal team members.
  • Ensure all documentation is complete, accurate, and compliant with organizational standards.

Required Qualifications

  • Previous experience in healthcare intake, patient engagement, enrollment coordination, medical administration, care coordination, home care, or a related healthcare support role.
  • Strong phone communication and relationship-building skills.
  • Ability to explain healthcare programs clearly, simply, and compassionately.
  • Comfortable handling appointment conversion and guiding caregivers toward scheduled visits
  • Experience using CRMs, EHRs, scheduling systems, spreadsheets, or referral tracking platforms.
  • Strong documentation and data management skills.

  • Bilingual communication skills are highly preferred.
  • Prior experience in healthcare enrollment or referral conversion roles is an advantage.
