Care Team Nurse
Job Overview
Our client is looking for a Psychiatric Registered Nurse to provide high-quality clinical
support and treatment monitoring within a telehealth environment. In this role, you will assist
patients with clinical inquiries, support treatment adherence, conduct standardized
assessments, and coordinate care with prescribing providers.
This position is ideal for an RN with strong communication skills, sound clinical judgment,
and a patient-centered approach. While psychiatric or telehealth experience is highly valued,
the primary requirement for this role is an active US Registered Nurse license.

Responsibilities
Clinical Patient Support & Treatment Monitoring
● Monitor and respond to patient clinical inquiries through communication platforms
and inboxes
● Assist patients with medication-related questions within scope of practice, including
side effects, dosing guidance, and treatment expectations
● Conduct standardized patient assessments and escalate clinical concerns when
appropriate
● Monitor patient progress, treatment adherence, and symptom updates
● Follow established escalation and patient safety protocols for high-risk or urgent
concerns
Patient Communication & Follow-Up
● Conduct patient follow-ups after medication adjustments or treatment milestones
● Educate patients on treatment plans, expected outcomes, and common concerns
● Provide compassionate, professional, and clinically accurate communication to
patients
Provider Collaboration & Care Coordination
● Collaborate with prescribing providers regarding patient updates and clinical
observations
● Coordinate with care team members to ensure smooth patient support and follow-up
● Maintain accurate and timely documentation of patient interactions and escalations
Administrative & Operational Support
● Assist patients with navigating the patient portal and telehealth systems
● Support process improvements and workflow optimization initiatives
● Participate in cross-functional collaboration to ensure a positive patient experience
Required Qualifications
● Active, unrestricted Registered Nurse (RN) license in the United States must qualify for the Compact Nursing License Alabama, Arizona, Arkansas, Colorado, Connecticut (Oct 2025), Delaware, Florida, Georgia, Idaho, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Maryland, Mississippi, Missouri, Montana, Nebraska, New Hampshire, New Jersey, New Mexico, North Carolina, North Dakota, Ohio, Oklahoma, Pennsylvania (July 2025), Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Vermont, Virginia, Washington, West Virginia, Wisconsin, Wyoming
● Minimum 2 years of nursing experience in a clinical setting
● Strong written and verbal English communication skills
● Ability to assess patient concerns, prioritize appropriately, and escalate when needed
● Comfortable using web-based platforms, EHR systems, and communication tools
● Strong attention to detail, organization, and documentation skills
● Able to work independently in a remote environment while maintaining
professionalism and responsiveness
Preferred Qualifications (Nice to Have)
● Experience in psychiatric nursing, behavioral health, mental health, primary care, or
telehealth
● Familiarity with standardized assessments such as PHQ-9, GAD-7, PCL-5, AUDIT,
DAST, or C-SSRS
● Experience using CRM, EHR, or telehealth platforms such as Kustomer or DrChrono
● Experience working in remote care coordination or virtual healthcare environments
● Exposure to ketamine-assisted therapy or behavioral health programs
