Responsibilities:
- Transform nurse visit information into accurate, complete patient chart entries
- Process OASIS documentation across multiple electronic health record systems
- Review and document medical imagery including wound photos as part of patient records
- Identify gaps or inconsistencies in documentation and proactively request clarification from nurses
- Maintain continuity by working consistently with assigned nursing teams and their patients
- Ensure all documentation meets strict HIPAA compliance and healthcare regulatory standards
- Participate in comprehensive training on home health-specific charting processes and requirements
- Build collaborative relationships with nurses to streamline their documentation workflow
Requirements:
- Strong attention to detail and critical thinking abilities
- Comfort level reviewing medical imagery including wound photography
- Ability to identify missing information and communicate professionally with healthcare providers
- Reliable internet connection and proficiency with computer systems
- Willingness to complete HIPAA compliance training (provided if not already certified)
- Bonus if you have experience with medical data entry, transcription, or healthcare documentation
